Support from the experts
We want to help improve your health and quality of life. When our members experience major health changes, we help them learn about their new care needs.
Our Care Management team is led by nurse care managers who work with a variety of specialists, like social workers and health coaches. This team will collaborate with your provider to offer you the tools and resources you need to manage your health. Care management is available to all our members at no additional cost.

Cardiac care management
The cardiac care management program is designed to help members living with or at risk for congestive heart failure, heart failure, coronary artery disease, or high blood pressure (hypertension). You'll get disease-specific education from a Cardiac registered nurse (RN) who can help coordinate your care. They can also provide assistance obtaining supplies like a blood pressure cuff, scale, etc., depending on your plan.
Diabetes
The diabetes care management program provides members living with or at risk for type 1 or 2 diabetes support from a Certified Diabetes Educator. They can also provide:
- Education by a Certified Diabetes Educator on American Diabetes Association guidelines on self-management practices, glycemic targets, and routine screenings/care
- Assistance in guiding positive lifestyle change, improving health, and treatment plan adherence through education, prevention, and early detection and treatment
- Promote member empowerment strategies and self-advocacy
- Care coordination (finding an in-network endocrinologist, dietician, or other specialist) and provider care plan support
Disease management programs
Our specialized disease management programs provide comprehensive support, education, and outcomes measurement for several conditions and diseases that frequently affect our members like arthritis, cancer, Crohn’s disease, cystic fibrosis, fibromyalgia, injuries, kidney disease, lupus, or multiple sclerosis.
Members with these conditions are identified and offered the opportunity to voluntarily participate in unique programming to meet the needs of individuals living with these conditions. Mass General Brigham Health Plan clinicians with expertise in these programs work to develop tools and materials to help members achieve improved health status and quality of life.
Respiratory care management
Our asthma and chronic obstructive pulmonary disease (COPD) program helps you better manage your respiratory conditions by making sure you get the care you need. A certified Respiratory Therapist will work with you and your health care provider to come up with a treatment plan that works for you. They can also provide:
- Support around new diagnosis or condition flare-ups
- Assistance with care coordination, finding in-network pulmonologist or other specialists
- Condition-specific education including the use of customized educational materials
- Assistance obtaining supplies like nebulizers, spacers, etc.
- Support creating an Asthma Action Plan
