Learn about your plan

 

The DoctorSmart directory lets you find and compare doctors, hospitals, and more. To get started, select your plan's network: Complete HMO2026 Choice Easy Tier HMO, or 2026 Choice Easy Tier PPO Plus.

If you don’t see your doctor in our network, let us know by contacting our Customer Service team. We'll double check for you. We can also help you find a new doctor in the network or talk with you about your options.

Virtual primary care
Virtual primary care is a good option to establish a new primary care relationship for comprehensive care where and when you need it. Learn more here.

With Optum Rx®, you get comprehensive prescription drug coverage. Read our FAQ to learn even more about what that includes.

We partner with Optum Rx to offer you great pharmacy benefits and a nationwide pharmacy network. You can search the drug list and get cost information using the links below.

During your switch over to Mass General Brigham Health Plan, we can help make sure you have access to the medications you need. Give our Customer Service team a call and let us know your prescription isn't on the drug list.

Your new ID card will arrive soon after you enroll, but you can view it online before it comes in the mail. After enrolling, you can visit Member.MGBHP.org and follow the steps to view or print your digital ID card. You can then show providers your card on your phone or download a printable copy.

If you are switching to Mass General Brigham Health Plan, let us know as soon as possible if you or a family member have care that needs to continue with your new plan by completing and sending this form (also available en español) to us by email, mail, or fax. If you have questions or need help, you can call us at 866-643-8392.

Our service team and nurses will help make sure you continue to receive care during your transition if you:

  • Have an upcoming treatment, procedure, or surgery planned
  • Are having a baby
  • Take prescription drugs that require authorization
  • Plan to see specialists with referrals already in place
  • Rely on durable medical equipment (DME) such as a wheelchair or sleep apnea device (CPAP)
  • Are undergoing care for an illness, chronic condition (like diabetes and asthma), your behavioral health, or special needs
  • Receive infusions at a hospital or home

YMCA of Greater Boston Choice Easy Tier HMO 1000 with Care Complement

A 3-tier hospital network plan, where the tier of provider you choose determines your out-of-pocket costs and you get full access to our broad provider network. You must select a PCP from our network, and referrals are required for most specialty care.

 

YMCA of Greater Boston Choice Easy Tier PPO Plus Combined 1000 with Care Complement

This plan offers access to top doctors and hospitals locally through Mass General Brigham Health Plan's 3-tiered network and nationally through the UnitedHealthcare Options PPO non-tiered network of over 1,600,000 providers and 5,600 hospitals. No PCP required and no referrals necessary.

 

Complete HMO HSA 2000 20%

This plan offers comprehensive care from a broad network of top providers—including Mass General Brigham and beyond. You must select a PCP from our network. Referrals are required for most specialty care. All services except certain preventive care are subject to the deductible.

Care Complement provides access to more affordable care options by removing cost sharing for certain services, therapies, and medications. Not available on the Complete HMO HSA plan.

Built for choice, savings, and convenience, FlexRx offers low-cost drug tiers for many medications, coverage for common over-the-counter drugs, and online tools to help you manage your plan and save money.
