MassHealth Drug List weight loss medication coverage changes

Coverage of weight loss medications is changing effective July 1, 2026.

The Mass General Brigham ACO is required by MassHealth to follow the MassHealth Drug List for all Mass General Brigham Accountable Care Organization (ACO) members. Beginning July 1, 2026, MassHealth will no longer cover drugs used for weight loss only.

Beginning July 1, 2026, weight loss medications will no longer be covered unless approved by your Mass General Brigham ACO plan, based on information provided by your doctor that shows a medical reason for use other than weight loss alone.

What do I need to do?
Please speak with your doctor. If you need to stay on a weight loss drug for a medical reason other than weight loss alone, your doctor is responsible for submitting a new pre-approval request to us.

Effective July 1, 2026, Wegovy® (semaglutide) will be the sole preferred drug to treat ALL other medically accepted conditions. The following conditions may be reasons for coverage of Wegovy® (semaglutide).

  • Moderate to severe obstructive sleep apnea
  • Certain liver conditions
  • A history of a heart attack, stroke, or symptomatic peripheral artery disease

If you have diabetes or prediabetes, your doctor will need to write you a new prescription for a diabetes drug.

Why are you changing your coverage of GLP-1s?

Mass General Brigham Health Plan’s coverage for MassHealth members follows state guidance for pharmacy coverage. Starting July 1, 2026, MassHealth will no longer cover medications used only for weight loss, including GLP-1s.

When will this policy update be in effect?

This coverage change is effective by MassHealth on July 1, 2026.

Can I still refill my weight loss medication prescription?

If you already have an approved prior authorization, you can keep filling your weight loss medication prescription until June 30, 2026, or when your prior authorization expires, whichever is sooner.

How will I refill my weight loss medication prescription after July 1?

Your current weight loss medication prescription will no longer be covered after July 1, 2026. If you need a prescription for a weight loss medication for another medical reason, please talk to your doctor.

How can I purchase a GLP-1 medication that is no longer covered?

The cost depends on several factors, including which GLP-1 medication you take. Some cost less than others, so check with your doctor or pharmacist about options.

How are you supporting members with their health and wellness goals?

We are pleased to offer comprehensive benefits to help members achieve their health and wellness goals. Our programs are designed for long-term success by meeting members where they are on their health journeys.

Where can members enroll in your health and wellness programs?
